Scheduled appointments and emergency care:

1. Contact the Community Practice on 466 3056 or 465 4161 Ext 145 to make an appointment..
2. Complete the Client Information and Consent Form and the Patient Information Form.
3. Submit the forms on line or print them. Do not sign the Consent Form at this stage. If you print the forms bring them with you to your appointment. Submitted forms will be available at the Community Practice at your appointment time.
